Customer Credit Application

Please complete all sections. This application is used to establish credit terms for feed and commodity purchases.

OWNERS / OFFICERS

Unless otherwise agreed in writing, all invoices issued by NTLS, LLC are due NET 10 from the invoice date. Invoices not paid within the stated terms may result in suspension of future shipments or credit privileges until the account is brought current.

Any balance not paid when due may be subject to a finance charge of 1.5% per month (18% annually) or the maximum rate allowed by law, whichever is less. Applicant agrees to pay all reasonable costs of collection incurred by NTLS, LLC including attorney fees, court costs,

and collection agency fees.

Applicant authorizes NTLS, LLC to obtain credit information from the bank and trade references listed above and from credit reporting agencies as necessary to evaluate this application and maintain credit privileges.

The undersigned certifies that the information provided is true and accurate and agrees to abide by the credit terms of NTLS, LLC.
